Julie Cruickshank was honoured with the accolade in a surprise ceremony this week on Monday, November 23.
She was praised for her tireless dedication to looking after her beloved mum, Linda Vance, and uncle, Albert Potts, who are both terminally ill.
Nicola Grayson, a family friend, nominated Julie for the award.
She told the Guardian: “Julie spends every day looking after Linda and Albert and hs never once complained, despite having her own illness and a teenage daughter, Millie, to look after.
“It’s an endless round of doctors and hospital appointments both local and out of town, such as Doncaster hospital for Linda’s chemotherapy.
“And Julie devotes her every waking hour to their care, often not getting home until way past midnight.
“She is an awesome lady and I know it is a dream come true for Linda and Albert to able to show her just how much they are appreciated with a Guardian Rose,” Nicola added.
